Findings

This mutation was found in a screen for variants in the open reading frame of the PSEN1 gene in participants from the U.S., Germany, and Canada who had been referred for AD diagnostic testing (Rogaeva et al., 2001). The cohort included 372 patients with AD and 42 asymptomatic individuals with a strong family history of AD. The S178P mutation was identified in two independent families. In one family, the mutation was reported to co-segregate with AD in two siblings, but unaffected non-carriers were not described. 

This variant was absent from the gnomAD variant database (gnomAD v2.1.1, July 2021).

Biological Effect

Amyloid β production was disrupted in HEK293 cells expressing this variant (Schultz et al., 2023). While the Aβ42/Aβ40 ratio was increased, the ratio of short to long Aβ species was decreased, indicating a damaging effect. An indicator of γ-secretase function as a percentage of wildtype activity was developed combining the Aβ (37 + 38 + 40) / (42 + 43) ratio—a measure of γ-processivity—with the commonly used Aβ42/Aβ40 ratio—a measure of the relative production of aggregation-prone Aβ. This composite score, 32.49 for S178P, was strongly associated with AD age at onset, as well as biomarker and cognitive trajectories across multiple PSEN1 variants. An earlier in vitro study with isolated proteins had revealed an increase in the Aβ42/Aβ40 ratio, although production of both peptides was considerably decreased in this assay (Sun et al., 2017).

S178 is not conserved between PSEN1 and PSEN2, but it is conserved between the PSEN1 genes of several vertebrates. Multiple in silico algorithms (SIFT, Polyphen-2, LRT, MutationTaster, MutationAssessor, FATHMM, PROVEAN, CADD, REVEL, and Reve in the VarCards database) predicted this variant is damaging (Xiao et al., 2021).
